Material weaknesses in financial crime controls
The assessment identifies six critical findings across customer due diligence, ongoing monitoring and outsourcing. Weak operating controls and incomplete records limit management’s ability to demonstrate effective oversight.
How each decision traces to the evidence
Hover or focus a record to trace it. Lines connect the cited source records, the recommendations that address each finding, and the recommendations each decision approves.
Assessment
The business-wide risk assessment does not sufficiently inform monitoring calibration. Customer file standards are applied inconsistently, and oversight does not reliably challenge the resulting exceptions.
Incomplete customer records weaken risk classification. Monitoring gaps increase reliance on manual reviews. Dispersed evidence and unclear ownership make it difficult for the board to confirm whether remediation has been effective.
The immediate priority is to limit exposure while controls are remediated. The programme should include enforceable interim restrictions, defined decision rights and evidence that can be independently verified.
Evidence required for reassessment
Reassessment should be based on file quality results, monitoring back-tests and documented board challenge, with independently verified closure evidence for each material finding.
Policy updates, training attendance and vendor attestations do not demonstrate operating effectiveness on their own. Validation should include a defined sample, exception analysis and independent approval.
Retain interim restrictions until evidence confirms that the relevant controls meet the agreed closure criteria.
